Dementia requires both a clinical/medical and a societal/community response. In delivering a personalised community support response one must consider integration between this and the rest of the ‘care and support ecosystem’ and the need for coordination. This is key to ensuring that people with dementia have access to a broad ‘care and support ecosystem’ that integrates community and voluntary organisations and mainstream services as well as health and social care services, offering the best opportunity to produce the optimal outcome for the person with dementia, their family and carers.
