Description: The philosophy of this project is to promote excellence, innovation and creative ways of supporting people with dementia to remain in their own home. Personhood is central to the well-being of the person with dementia and care must be focused on empowering people with dementia to achieve their aspirations. Maintaining independence and quality of life is of key importance to people with dementia. This can be achieved through signposting to existing services and providing information tailored to individual's needs. Access to a holistic, personalised support plan which is continually reviewed and adapted is a fundamental component for each individual's journey. A proposed Dementia Advisor will be central to enabling statutory, voluntary and community agencies to work collaboratively and will consider the desired aspirations and outcomes of people with dementia, rather than simply looking at care supports needs in the narrowest sense. This will provide the flexibility and imagination required to transform the culture of dementia.
