Development of community alternatives to congregate/institutional care

Applications to fully or partially close an institution/congregate setting and to develop community alternatives to institutional/congregate care will be assessed according to the following criteria:
Individual-level criteria:
1. Involvement of the individual in planning the design of the services/supports they require
2. Individualisation of services and supports
3. Enhancing community inclusion by:
- Identifying the person’s strengths and potential contributions in the context of their neighbourhood/community
- Supporting the person to undertake roles valued by them and the community
- Providing as many opportunities as possible for the person to develop relationships with those not paid to spend time with them
- Using mainstream public services, community resources and employment and leisure options
4. Promoting collaboration between individuals and agencies that can assist the person to achieve their goals by involving
- Families, neighbours, friends, advocates and others as appropriate
- Other agencies, particularly those that are non-disability/mental health-specific
Organisational-level criteria:
5. Development of community alternatives leading to full or partial closure of an institution/congregate setting
6. Alternative accommodation provision characterised by the:
- separation of provision of supports and services from the provision of accommodation
- person having his or her own tenancy
- person choosing who they live with (regardless of whether or not they have a disability or mental health difficulty) and where they live
- accommodation of not more than four people with a disability/mental health difficulty
- accommodation in the community i.e. in typical residential settings (rural/urban), not in the grounds of disability or mental health services
7. Use of support staff/recovery guides with appropriate competencies from a variety of backgrounds
8. Availability of matching funding and/or resources to that sought from the Genio Trust.
9. Cost-effectiveness and value for money
10. Financial sustainability beyond the life of the grant
11. Capacity within the organisation to implement proposal
